For my capstone project, I am considering a medium-sized community health center in an urban setting. This health center serves a diverse population, including individuals from low-income backgrounds and marginalized communities. The primary services provided by the health center include primary care, preventive care, and health education programs.

At this stage, I have not yet found a mentor for my capstone project. However, I am actively exploring potential mentors within the health center who can guide and support me throughout the project (Fernandez-Fernandez et al., 2022). A mentor’s role in the facility would typically involve providing expertise in the field of healthcare quality improvement, offering guidance on data collection and analysis, and assisting in the development and implementation of improvement strategies.

As for my tentative capstone topic, I am interested in exploring the effectiveness of patient education programs in improving health outcomes and promoting preventive care utilization. I would like to assess the existing patient education initiatives at the community health center, identify any gaps or areas for improvement, and propose strategies to enhance the impact of these programs.
